All athletes must have a current physical exam on file in the office before turning out for a team. Physical exams are considered current for two years from the date of the exam. This WIAA Form is the preferred physical form, but we also accept physical forms from pediatrician offices as long as they state that the athlete is cleared to play sports.
All athletes must also have a current (no older than 24 months) baseline concussion test on file in the office before turning out for a sport. We are offering concussion baseline testing to our 2022 fall athletes on Wednesday 8/17, Thursday 8/18, Friday 8/19, Monday 8/22.
