Please read this note on Physical Exams and Baseline Concussion Testing:
All athletes must have a current physical exam on file in the office before turning out for a team. Physical exams are considered current for two years from the date of the exam. This WIAA Form is the preferred physical form, but we also accept physical forms from pediatrician offices as long as they state that the athlete is cleared to play sports.
All athletes must also have a baseline concussion test on file in the office. For fall 2020 only, we are waiving the two year expiration of baseline concussion tests, so if an athlete has a test of file, it will be considered current.
If an athlete has no concussion test on file, concussion testing will be offered in August. The concussion baseline test takes about one hour to complete and only 30 students can test at one time. Please note that lines have been long. Fall Concussion Baseline Testing has been tentatively scheduled for Aug 17 12:00-2:00pm, Aug 18 12:00-2:00pm, Aug 19 4:00-6:00pm, Aug 20 4:00-6:00, Aug 21 4:00-6:00pm, and Aug 24 at a time TBD.
