Washington State law (RCW 28A.210.370) requires that all students who have been diagnosed with asthma must have the needed medical permission forms, medication and/or equipment, and a nursing care plan in place BEFORE THE STUDENT IS ALLOWED TO ATTEND SCHOOL.

The following information must be submitted to the School Nurse if you have a student who has asthma. 

• Asthma Action Plan completed by Health Care Provider and parent.
• Medication to be given at school must be brought to the Health Room in the original container prior to attending school
